The 'Vsix' package did not load correctly

The 'Vsix' package did not load correctly

Tried uninstalling and reinstalling, but failed to work.

Seem unable to attach my activity log to this form as how ever many times I try, "Jira could not attach the file as there was a missing token. Please try attaching the file again."
