Create New QTGui class fails if Form UI file already created

Initially created new UI form using Add New Item/QT Forms/QT Widget from the right click popup menu. Created form named customWidget.ui.

Realized h and cpp files were not created. Then tried to add files using add new class. I would then get the error messages on the attached jpg files.
