permalinks for topics do not work

Permalink generation for topics are not generating valid links - e.g. see http://codereview.qt-project.org/#/t/13/ if you click "copy permalink to clipboard" button it gives you http://codereview.qt-project.org//topic,13/ but if you try to follow that url, it give 404 error.
