We put VMs into INVESTIGATE_ME state quite often. I just observed one instance that was stuck in the "PROLOG" state. After doing a "recover-retry" I had a working VM which launched the agent.
I propose attempting to recover machines more often, of course with a limited number of attempts. This may save resources. Of course it would be great to know why the operation failed.
