samplephonebook: Error shows after launch application, no response when pressing "Add" button

Reproduction steps:
1) Launch "samplephonebook" application.
2) Press "Add" button..

Expected result:
Samplephonebook application launches properly, "Add" button could be pressed to add new contact

Actual result:
Error message shows after open application, "Add" button is not availble when pressing it
