QtXmlPatterns: Add pull interface

Since one is only supposed to use QXmlNodeModelIndex for custom data models more or less, that leaves only a QAbstractXmlReceiver as the viable method for introspecting the hierarchy of output. For that reason a pull interface would be useful. It would essentially be the internal class AbstractXmlPullProvider.
