QOAuth2AuthorizationCodeFlow stuck in RefreshingToken state

      When you execute refreshAccessToken() on QOAuth2AuthorizationCodeFlow and the request fails (e.g. the server may be temporarily unavailable), the class is stuck in the RefreshingToken state. You can't try to refresh again because the method forbids it when it's already refreshing.
