Create a pure Word template

Create a pure word template that is a master for all QSR docs.

Now there are simply too many styles in the document that are burden.

• Create one template for A4 portrait only
• Create one template for A4 mixed portrait and landscape (different sections)

This will help to survive in the documentation jungle.
